Health in Mind are delighted to launch our new online self referral function onto our website. Enabling you to access support directly without having to first have an appointment with your local GP or other health professional.

 

Here's what you need to know when making your referral...

 

Who can refer?

We provide courses and therapies to people who are registered with a GP in East Sussex (excluding Brighton and Hove), are over 18 and struggling with stress, anxiety and or low mood.

 

How can I refer? 

There are a number of ways to make a referral:

  • If you want to speak to someone first about the treatment options available, the quickest way to make a referral without delay is to do this online using the link below. 
  • If you prefer, you can still be referred by your GP or another health professional. You can ask them to support you with this.
  • If you are only interested in attending one of our Stress Control Courses you can find further details of these courses and registration forms on our courses page. You will not need to complete a full referral form via the link below.

 

What else do I need to know?

If you are already receiving support from our service or you have already completed a self-referral via a different route, for example with your GP please do not submit either a new referral or complete a course registration form.

Again, if you are only interested in attending the Stress Control Course please do not complete a self-referral form, go to the courses page instead.  This is because it may slow down the process and take longer for us to contact you if we receive multiple referrals from you.  

 

What happens when I refer online?

The self referral form has been designed to be easy to understand and gather all the information we need to arrange your first appointment.

Once complete, your referral will be live on our clinical system and means we'll be able to provide you with a response and the help you need sooner.

Once you have completed the referral, a member of the Health in Mind team will contact you.

Please note we are not a priority service and may take up to 20 working days to get in touch. If you need urgent support please use the numbers at the bottom of this page.

 

What happens to my information?

Please be assured that this process is totally secure and complies with data protection and information governance requirements.

Please be aware that we will let your GP know that you have accessed Health in Mind. If you do not want this to happen, please let us know on your referral form. If you would like to know more about our confidentiality policy please use this link: Confidentiality policy
