About the East Sussex Triage HUB
There may be times when it seems another service may be considered more suitable for your individual needs.
Our service is part of a local Triage HUB, a partnership of local health providers working together to make sure that you access the service which is right for you.
On rare occasions, in order to connect you to the right service, the triage HUB may pass your referral to one of our partner services for them to contact you. This does not impact your data protection rights.
The following services are partners in the Triage HUB:
- NHS Assessment and Treatment Service
- Change, Grow, Live (Drug and Alcohol Support Service)
- Adult Social Care
- Southdown Support Services
When completing your referral to our service, you will be asked to say whether you are happy for your referral to be reviewed by the East Sussex Triage Hub.
If you do not consent to this, your referral will still be processed without delay and be reviewed by our Health in Mind team.
