Your appointment with Health in Mind

Thank you for booking your appointment with Health in Mind. Your appointment details were confirmed through the online choose and book system. You will be sent a reminder prior to your appointment.

This is for an initial assessment where we will review with you your current circumstances and discuss what support might be appropriate.

Sussex Partnership Foundation Trust is a training organisation therefore your appointment could be with a clinician in training.

We want to make our service as easy to access as possible and as a team we are always thinking about whether we can improve what we do. The Health in Mind team would really like to hear from you about your booking experience and what we can do to make it better. How to attend your appointment

Here is some further information about your appointment and how we will contact you:

  • Telephone appointments - If you booked a telephone appointment our team will call you on the phone number provided. This call will be from a withheld number.

Before the appointment

  • To make the most of your time you will need to answer some questions before your appointment, these are to monitor how you are doing and to make sure we can provide you with the appropriate support.
  • You will need to access our Patient Portal to complete these questions. The Patient Portal can be accessed from this link: https://tx.vc/r/1lYPv
  • To log in you will need your personal secure ID, this will be sent to you via text and contains 9 numbers. 

What to do to prepare for your appointment

  • Please complete the questionnaires that we will send to you via the patient portal.
  • Take a look at, and complete the Service Equality Monitoring form in advance of the session - the team member will ask you these questions in your assessment appointment. If you have any questions regarding your appointment or need to cancel/rearrange, please email us at SPNT.healthinmind@nhs.net or alternatively telephone us on 0300 00 30 130.

Our main office is open Monday – Friday, except bank holidays, 9:00am to 5:00pm

If you miss or cancel an appointment we are able to offer an alternative one but there may be a wait. However, if you cancel or miss a second appointment during your treatment we may not be able to offer you any further appointments. We appreciate that the reason for a cancellation or not attending an appointment is often due to an unforeseen event, however, we are unable to offer open ended appointments. Thank you for your understanding.
